"There’s a 1 in 5 chance that an older patient who has had a common surgery will end up in the emergency room within 30 days of their hospital stay, according to a new study in Health Affairs. Transitions from hospitals to the outpatient setting can be dangerous for many older adults. As patients are still recovering when they are discharged from the hospital, they might not understand or remember instructions about caring for themselves at home.
